CARE NEW England on Thursday said it has received one of 32 national Accountable Health Communities model grants from the Centers for Medicare & Medicaid Services, receiving an anticipated $3.9 million for a five-year initiative focused on ensuring that high-risk Medicare & Medicaid patients have access to high-quality health care services.

PROVIDENCE – The Centers for Medicare & Medicaid Services recently approved Integra Community Care Network, the Care New England-affiliated physician network, as one of 89 new Medicare Shared Savings Program Accountable Care Organizations.

Founded in 2014, Integra becomes the third ACO doing business in Rhode Island, joining Coastal Medical and Southcoast Health.

Overseeing the new ACO is Domenic Delmonico, who is acting director of the Integra Community Care Network and senior vice president for managed care contracting and network development at Care New England.

The goal of ACOs is to coordinate care of patients, ideally both lowering costs and improving care. Nationally there are 405 ACOs in the Shared Savings Program, as of January 1.

ACOs share in the savings that they generate for Medicare after meeting quality targets.
