Care Resource program targets congestive heart failure

PROVIDENCE – Care Resource recently announced the creation of a “residentialist” program catered to the needs of individuals suffering from congestive heart failure.
The program features a nurse practitioner responsible for conducting home visits for patients admitted to Care Resource. The nurse practitioner also leads the clinical team at patient care rounds and collaborates with the patient’s primary care physician to manage the patient at home.
The program is structured to lower hospital readmission rates and avoid emergency department visits for patients identified as high risk. The nurse is licensed to provide infusion therapies for congestive heart failure patients, such therapies including Lasix, Dobutamine, hydration and antibiotics.
