Collaborative meets, launches pediatric care expansion

PROVIDENCE – Members of the state of Rhode Island’s patient-centered medical home initiative recently met to discuss the group’s performance in the past year as well as the expansion of its purview to pediatric care.

Formerly known as the Chronic Care Sustainability Initiative, the group is now known as the Care Transformation Collaborative (CTC) of Rhode Island. Under the old name, the group has been operating since 2008.

The collaborative currently includes 73 participating practice sites, serving more than 300,000 Rhode Islanders. The decision to include 10 pilot pediatric practices was made at the meeting. Under the plan nearly 30,000 children in the state would gain access to the patient-centered medical home model.
