PROVIDENCE – The Rhode Island Chronic Care Sustainability Initiative has expanded to include an additional 20 practices, increasing the number patients served to more than 260,000 Rhode Islanders in a patient-centered medical home model, according to initiative officials.
The expansion, which was announced in April, will take effect on Oct. 1.
“Our open call for applications saw a tremendous response rate, evidencing that primary care practices in Rhode Island value and want to participate in this new and effective model of care,” said Debra Hurwitz, co-director of CSI-RI. “We’re pleased to welcome 20 new practices to our initiative, providing them access to new tools that will help improve the health of their patients and our state’s health care system.”
Practices accepted into the initiative include: Anchor Medical Associates (Lincoln, Providence and Warwick), Aquidneck Medical Associates (Newport and Portsmouth), Associates in Primary Care (Warwick), Comprehensive Community Action Program (Cranston, Coventry and Warwick), East Bay Community Action Program (East Providence), Family Medicine at Women’s Care (Pawtucket), Internal Medicine Center (Pawtucket), Internal Medicine Partners (North Providence), Medical Associates of RI (Barrington and Bristol), Nardone Medical Associates (Pawtucket), Ocean State Medical (Johnston), Richard Del Sesto (East Greenwich), South County Walk-in and Primary Care (Narragansett), Thundermist Health Center (West Warwick), Tri-Town Community Action Program (Johnston), University Internal Medicine (Pawtucket), University Medicine (East Providence, Providence, and Warwick), WellOne Primary Medicine (Foster, North Kingston and Pascoag), Women’s Primary Care, Women’s Medicine Collaborative (Providence) and Wood River Health Services (Hope Valley).
CSI-RI launched its first five pilot sites in 2008, added eight practices in 2010 and another three in October 2012.
In addition, a new initiative, CSI-Kids, to focus on pediatric practices, is in the process of being launched.
