South County Medical Group East Greenwich earns national recognition

EAST GREENWICH – The South County Medical Group East Greenwich Family Medicine practice announced that it received the Patient-Centered Medical Home Recognition from the National Committee for Quality Assurance, for its use of evidence-based, patient-centered processes focusing on highly coordinated care and long‐term, participatory relationships.

To earn recognition – valid for three years – South County Medical Group Family Medicine demonstrated the ability to meet the program’s key elements, which embodies characteristics of the medical home and combines teamwork and information technology to improve care and patients’ experience and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, rather than approaching patient care solely through episodic office visits, according to the South County Medical Group Family Medicine statement. Each patient’s care is overseen by clinician-led care teams who coordinate treatment across the health care system; research has shown that medical homes can lead to higher quality and lower costs, as well as patients and providers reporting higher satisfaction with the care delivered, the statement noted.

“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” NCQA President Margaret E. O’Kane said in the statement. “Recognition shows that SCMG Family Medicine has the tools, systems and resources to provide its patients with the right care, at the right time.”

NCQA standards aligned with the joint principles of the Patient-Centered Medical Home, which were established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.

A South County Health entity, SCMG Family Medicine includes family medicine and internal medicine physicians, and offers patients seamless continuity of care, if a specialist or hospitalization is necessary. Family medicine physicians care for patients of all ages, from newborns to geriatric patients, while internal medicine physicians care for adults.
