Neighborhood Health Plan of Rhode Island has embarked on a new community based outreach and intervention program aimed at improving health outcomes for children with asthma.
The program will involve three main components: care coordination, community outreach and dissemination of up-to-date practice guidelines, according to Rep. Patrick J. Kennedy, who joined officials from the health organizations Monday in announcing the new program.
“Beating Asthma,” funded through a $250,000 federal grant, also be organized and staffed by a number of community health providers, including Blackstone Valley Community Health Care in Pawtucket and Thundermist Health Center in Woonsocket and Wakefield.
“The Beating Asthma program expands the locus of asthma care beyond the walls of a provider’s office and into the community,” said Dr. Renee B. Rulin, chief medical officer at Neighborhood Health.
