Care New England plans to use the funds to create an internal and public-facing “dashboard” of social drivers of health-facing primary care populations.
“Our goal is to summarize the Social Drivers of Health data collected in [the] CNE/Integra primary care setting, provide analyses relevant to community stakeholders and make a select dataset available to internal partners and the public in support of efforts to improve SDoH conditions in Rhode Island,” said Dr. Joseph Diaz, chief health equity officer at Care New England and associate dean in the Office of Belonging, Equity, Diversity & Inclusion at Brown University’s Warren Alpert Medical School.
