PACE-R.I. joins CareLink network

PROVIDENCE – The PACE (Program of All-inclusive Care for the Elderly) Organization of Rhode Island today announced it has joined CareLink, a nonprofit care management organization.

PACE-R.I., founded in December 2005, is a member of the National PACE Association. It offers an array of services – from transportation to medical appointments, to adult day care, to coordination of care – to help frail elders remain independent. Its services are available to people 55 and older who live anywhere in the state but Block Island or Prudence Island, and who would qualify for nursing home care but prefer to remain in the community.

CareLink is a provider of management support, administrative, program development, planning and group contracting services to its member agencies. This month, it marked its 10th anniversary as a long-term care management organization.

Besides PACE, CareLink’s members include the St. Antoine Residence, Steere House, the Scandinavian Home, Cornerstone Adult Services, the Jewish Seniors Agency of Rhode Island and Home & Hospice Care of Rhode Island, among others.
