CSI-RI participants to share experiences at Jan. 29 forum

CRANSTON – The Rhode Island Chronic Care Sustainability Initiative (CSI-RI), a collaborative effort to remodel local primary care practices as “patient-centered medical homes,” is hosting a forum Jan. 29 to share its experiences and discuss the future.
The event, from 8 a.m. to noon at Rhodes on the Pawtuxet, is free and open to the public.
The pilot practices – University Medicine Foundation, Coastal Medical, Family Health and Sports Medicine, Hillside Avenue Family and Community Medicine, and Thundermist Health Center – will give presentations with provider and patient perspectives.
In addition, the forum will cover the history and purpose of CSI-RI, how it will be evaluated, what the future holds and how the project fits into the broader health care reform picture.
