Southcoast Visiting Nurse Association renamed Southcoast Health at Home

FALL RIVER – Southcoast Health’s home health and supportive care organization, formerly known as the Southcoast Visiting Nurse Association, will operate under its new name Southcoast Health at Home, the health system announced.

The change reflects the organization’s growth, continued services and dedication to providing care for patients at home, according to Southcoast Health.

Fernanda Johnson, Southcoast Health’s director of quality, compliance and education, has been appointed as Southcoast Health at Home’s new vice president.

“For 120 years, our team of caregivers has met our neighbors with one mission in mind – bringing comfort, healing and compassion to the south coast community,” Johnson said. “We are proud to be an essential member of the Southcoast Health family, providing trusted and sensitive care to patients where they feel most at home.”

Southcoast Health at Home was named Community’s Choice for Best Home Health Care Services for the eighth year in a row. The clinical team includes physicians, nurse practitioners, nurses, therapists, social workers and certified nursing assistants.

Services offered include home health care to help patients manage illness or injury at home; specialized care in cardiac, post-joint replacement recovery and other specialties; supportive care like palliative and hospice care; symptom management and support for patients facing serious or life-limiting illness; and bereavement support for those coping with the loss of a loved one.
