PORTSMOUTH – The Rhode Island Foundation has granted the independent nonprofit Visiting Nurse Home & Hospice $80,200 for pharmacist services provided by its Home Care Pharmacy program.
Visiting Nurse Home & Hospice cares for people of all ages with chronic and terminal illnesses at the end of their lives. The care includes the addition of a pharmacist on the nonprofit’s Home Care Team as part of its Home Care Pharmacy program, according to a statement from the organization.

The pharmacist service, which is not covered by medical insurance programs, offers personalized medication management to help educate patients on how to self-administer their medications, reconcile various prescriptions that the patients have in their homes and consult with physicians about new or changed prescriptions once the patients return home from the hospital.
“Ensuring everyone has access to the care they need in order to live healthy lives is one of our priorities. By encouraging delivery in new, creative ways, we can make a significant difference in the lives of patients,” said Larry Warner, the Rhode Island Foundation’s strategic program officer in the health sector.
The Home Care Pharmacy program has provided health care savings for the patients seen by the pharmacist and has contributed to lower rehospitalization rates for patients, Visiting Nurse Home & Hospice stated.
“We are committed to offering the best practices of home health care to the community, whether the existing insurance industry reimburses for the service or not. Visiting Nurse Home & Hospice is extremely grateful to the Rhode Island Foundation for its support in our continuing with the home care pharmacist program,” said Jennifer Fairbank, CEO of Visiting Nurse Home & Hospice.
