Department of Health releases report on hospital quality

The Rhode Island Department of Health yesterday released a report on ten indicators of quality hospital inpatient care in Rhode Island. The report uses administrative data for the period 1994-1998. It compares the combined performance hospitals in Rhode Island with naitonal benchmarks-identical performance meaures for a sample of hospitals across the country.

The ten quality indicators came from a set of 35 performance indicators developed by the Agency for Healthcare Research and Quality, a federal agency leading the quality improvement process in health care.

The four areas where Rhode Island hospitals performed above the national benchmark include:

  • Pulmonary compromise (lung congestion, difficulty breathing) following major surgery
  • Gastrointestinal hemorrhage or ulceration (bleeding or hole in the lining of the stomach or bowel) after major surgery
  • Venous thrombosis or pulmonary embolism (blood clots in the veins or lungs) after major surgery
  • Obstetrical complication (bleeding or severe tears after delivering a baby, etc.)

    For three indicators, hospital performance appeared similar to national benchmarks:

  • In-hospital mortality following cholecystectomy (gall bladder removal)
  • Urinary tract infection after major surgery
  • Pneumonia after invasive vascular procedure (angioplasty, pacemaker, etc.)

    In the remaining three areasm hospital in Rhode Island did not look as good as the national benchmarks:

  • Acute myocardial infarction (heart attack) after major surgery
  • Wound infection
  • Adverse effects and iatrogenic complications (complications resulting from medical care, such as gastrointestinal or heart complications)
