•Lisa Crevier has been promoted from Office Services Assistant to Administrative Assistant

As senior healthcare analyst, Blake works with project teams in support of their data needs, providing analytical support to the Patient Safety, Safe Transitions and Chronic Kidney Disease teams.

As administrative assistant, Lisa supports our work on Safe Transitions, furthering Rhode Island’s leadership around care coordination. The project:

•focuses on patients’ discharges from the hospital to other care settings,

•promotes cross-setting communication, and

•ultimately, aims to improve patients’ transition experiences, self-management skills and outcomes.

As part of our work on the 9th Statement of Work with the Centers for Medicare & Medicaid Services (CMS), Quality Partners of Rhode Island works with physician offices, nursing homes, hospitals and home health agencies to improve the quality of care received by patients.
