Medicare, the federal health
insurance program for the elderly and disabled, reduced its
payment-error rate to 5.8 percent in fiscal 2003, down from 6.2
percent in the previous two years, according to a report.
Medicare made $11.6 billion in payments for services that
were medically unnecessary, inadequately documented, or
improperly coded in the last fiscal year, ended Sept. 30, the
Centers for Medicare & Medicaid Services said in an e-mailed
statement.

The agency began an annual survey of 6,000 claims in 1996 to
determine the rate of improper payments to doctors and
contractors. This year, CMS increased the number of claims
reviewed to 128,000 to get more information about how and where
errors are made, said Tom Scully, CMS Administrator.
