Lynne Chase has been named program administrator for the safe-transitions team at Quality Partners of Rhode Island. Chase has been working for Quality Partners of Rhode Island since 2008 as a program coordinator for the CMS Medications Measures Special Study, as well as in a supporting role with the Chronic Kidney Disease team. Previously, Chase served as project manager for CVS Caremark in Woonsocket. •
