Lynne Chase has been named program administrator for the safe transitions team at Quality Partners of Rhode Island. In her new role, Chase will provide day-to-day leadership for the safe transitions project, which focuses on improving coordination across the continuum of care. Chase has served as program coordinator for the CMS Medications Measures Special Study since 2008. •
